Welcome to Our Client Portal

Our Client Portal is a secure online platform designed to provide you with convenient 24-hour access to certain aspects of your personal health information and to facilitate communication with your healthcare team for non-urgent issues only.

By using the Patient Portal, you agree to the following terms and conditions:

1. Not for Emergencies

DO NOT use the Client Portal for urgent or emergency situations. In the event of a medical emergency, call 988 or go directly to the nearest emergency room.

2. Medical Advice Disclaimer

The information available through the Portal, including test results and educational materials, is provided for informational and educational purposes only. It is not intended to be a substitute for professional medical advice, diagnosis, or treatment. You should always seek the advice of your provider or other qualified healthcare provider with any questions you may have regarding a medical condition or treatment.

3. Security and Confidentiality

  • Your Responsibility: You are responsible for maintaining the confidentiality of your username and password and for all activities that occur under your account.
  • Secure Environment: The Portal uses encryption to protect your health information. However, as information is transmitted over the internet, we cannot ensure or warrant the security of all information transmitted.
  • Safeguarding Access: Do not share your login information with anyone you have not formally authorized through a proxy agreement.

4. Communication Guidelines

  • Response Times: We will make reasonable efforts to respond to Portal messages promptly, typically within 2-3 business days. The Portal should not be used for time-sensitive matters.
  • Appropriate Use: Use the Portal for communications regarding your own health care needs and treatment. Do not use it for unlawful purposes or to transmit harmful or malicious content.
  • Inclusion in Medical Record: Communications through the Portal may be included in your permanent medical record.

5. Your Information

You can access certain health information, such appointment times and documents, through the Portal. Please note that the Portal is not your complete medical record. For your complete medical records, you may need to submit a formal request.

6. Termination of Access

We reserve the right to terminate or suspend your access to the Portal at any time for any violation of these terms.

By proceeding to use the Client Portal, you acknowledge that you have read, understand, and agree to these terms and conditions. Your use is voluntary and will not impact the quality of care you receive.
