Rates & Insurance

Live Well Counseling Center accepts most insurances, including the following: Aetna, Ambetter, Blue Cross/Blue Shield, Medica, Medicaid and all Medicaid HMO’s-(United Healthcare Community Plan, Nebraska Total Care and Molina Healthcare), Midlands Choice Network, United Health Care – Commercial and Private Plan. LWCC is not in network with Medicare therefore cannot accept someone who has Medicare who has secondary insurance. This is also with someone who have dual Medicare/Medicaid.

Services may be covered in full or in part by your health insurance or employee benefit plan. We recommend that you contact your insurance company prior to your first appointment to verify your coverage and benefit information.

Payments: Cash, check, and credit/debit cards are all forms of accepted payment. A copy of your insurance card will be requested at the initial intake assessment. All copays and private pay are due at the time of service.
